At Delaware First Health, we understand much of what impacts health happens outside of the doctor’s office. We are focusing on health and wellness and health equity for all members to promote successful health outcomes.

We strive to meet our members where they are through our Community Connections program. We connect our members to needed resources from food, housing, employment, and education.

We develop strategic partnerships with community-based organizations to ensure our members have access to receiving services.
