Healthy Activities Eligible for Rewards

$10 Member Portal Registration. One time reward.
$15 PCP Confirmation within 30 Days of Enrollment. One time reward.
$20 Health Risk Assessment. One time reward.
$20 Child Well Visit Ages 2-18. Once per year.
$15 Adult Well Visit. Ages 18 and up. Once per year.
$15 Flu Vaccination. Ages 6 months and up. Once per year.
$20 Annual Adult Dental Visit. Once per year.
$20 Diabetes HbA1c Test. Ages 18-75. Once per year.
$20 Notification of Pregnancy. One per pregnancy.
$50 Prenatal Visit in 1st Trimester or within 42 days of enrollment with DFH. One per pregnancy.
$20 3rd Prenatal Visit. One per pregnancy.
$20 6th Prenatal Visit. One per pregnancy.
$40 Postpartum Visit. One per pregnancy.
$100 Infant Well Visits Ages 0-15 months. One time reward. Requires six visits or claims to receive reward.
$25 Lead Screening at 12 Months. One time reward.
$25 Lead Screening at 24 Months. One time reward.
$15 Breast Cancer Screening. Ages 40-74. Once per year.
$15 Cervical Cancer Screening. Ages 21-64. One time reward.
$15 Colorectal Cancer Screening. Ages 50-75. One time reward.
$20 1st Tobacco Cessation Medication Fill. Once per year.
$20 Behavioral Health Hospitalization Follow-up Visit. Once per year.
$20 Substance Use Disorder Residential Stay Follow-up Visit. Once per year.
*To be eligible for this reward, you must notify us you are pregnant prior to having your baby by submitting a completed Notification of Pregnancy (NOP) form.
Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays™ Visa® Prepaid Card will be mailed to you.
This card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted. See Cardholder Agreement for complete usage restrictions.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. You will receive your card approximately three months after you complete your first qualifying activity. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact us at 1-877-236-1341 (TTY: 711).
Once you receive your My Health Pays™ rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.
To check your balance, login to your Secure Portal account.
Your rewards will be added to your card approximately two months after you complete the qualifying activity. You will receive your card with your first reward approximately three months after you complete your first qualifying activity.
Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
